This is our 3rd year recognizing service improvement through the Service Breakthrough Awards.  We have amended the process to comply with the impact of HCAHPS.  There are 10 categories in HCAHPS and 8 categories in traditional Service Breakthrough.

The 2008 Service Breakthrough Awards Ceremonies and Luncheon will take place on Thursday, November 6 in Louisville, Kentucky at the 9th Annual HealthCare Service Excellence Conference.

When you have finished filling out the nomination form you may either:

• Upload your nomination form directly to the Summit Awards Committee:
  • Save each nomination on your computer in the correct format described in the form

  • Submit your nomination using the Upload Page      OR
     

• Print out and then Fax your scores or nomination to Summit Nominations at 403-228-6776
